Birmingham and Solihull ICS Digital Weight Management Programme

Join us to help patients across your locality adopt life-long healthy habits. Birmingham and Solihull ICS have partnered with Ardens and iPLATO  to encourage eligible patients to self-refer via SMS/Letter to a Weight Management programme. 

Ardens is a trusted NHS provider specialising in digital health services.  This is a fully funded NHS initiative with no cost to the patient or your practice. 

The programme offers a 12-week online behaviour and lifestyle programme with remote personalised support and self-led learning to people with smartphone or computer with internet access. The programme is designed to support adults living with obesity who also have a diagnosis of diabetes and or hypertension to manage their weight and improve their health. 

Why should I get my practice involved?

As healthcare professionals, we have a shared goal to advance healthcare as we know it and make positive change happen for others.

This is a fully funded initiative at no cost to your practice and there is no noticeable increase on practice workload.

Registering your practice ensures your patients will receive texts with GP endorsement which is more effective than no GP endorsement.

It’s simple and easy to support:

Step 1: Register your practice.

Step 2: Have a 5-minute set-up call (if required).

Step 4: Send in a list of patients who have opted out of receiving text messages.

Step 3: We do the rest.

How will it work?

  • Once practices have consented, they will need to provide a list of eligible patients through NHS mail.
  • iPLATO will send an initial SMS inviting patients to provide consent to join the programme. 
  • Patients that do not act upon the initial SMS will receive a letter and a follow up SMS.
  • Patients that provide consent will allow practices to see the details of eligible patients who wish to be referred onto the programme and complete the e-referral process currently in use.
  • Patients that provide consent via the link will have their details available to practices via the myGP Connect portal. This efficient method will allow practices to see the details of eligible patients who wish to be referred onto the programme and complete the e-referral process currently in use.  
  • This SMS Programme will be in addition to any existing programmes.

Data Processing and Data Sharing Summary

  • Practices will provide the NHS numbers, First Names, Last Names of the patients to be targeted and clinical system search results will be sent via NHSmail to iPLATO.
  • To provide the Services, the Personal Data will be subject to automated and manual processing operations by iPLATO, including collection, use, transfer, storage and erasure.
  • The Personal Data processed by iPLATO under this Agreement are not transferred by iPLATO to anybody unless expressly instructed by the GP Practice / Healthcare organisation.
  • iPLATO Healthcare acts as a Data Processor for this service and will Process the Patient Data only to the extent, and in such manner (with due regard to any technical constraints) as is necessary for the provision of services to the GP Practice / Healthcare organisation
